Use this form if an item had been fitted and then failed while the vehicle was being driven, operated or otherwise used.

We aim to respond within 24 hours

We often respond much sooner during normal working hours. Responses may take longer at weekends, on bank holidays or during particularly busy periods.

This form is for items that failed after installation

Complete this form where the item had been installed and initially operated, but later developed a fault or failed while the vehicle was being driven or used.

Full vehicle details

Include the full model, engine, year, UK registration number and complete 17-character VIN where possible.

Installation details

Tell us who fitted the item, when it was installed and whether any coding, calibration, adjustment, bleeding, priming or alignment was carried out.

Please give a clear timeline from fitting through to the point when the problem occurred.

Time and mileage in use

State approximately how long the item had been fitted and how many miles the vehicle had covered before the problem appeared.

Conditions at the time

Explain whether the vehicle was starting, idling, braking, steering, accelerating, towing, travelling off-road or being driven normally when the fault occurred.

Events before the failure

Tell us whether there had been any earlier noises, vibration, warning lights, leaks, intermittent operation, overheating, looseness or other unusual symptoms.

Please Describe the Exact Symptoms

Do not simply state that the item is “faulty”, “broken” or “does not work”. We need enough detail to understand what happened and how the suspected fault was identified.

Describe any noises, vibration, excessive movement, leaks, warning lights, fault codes, loss of pressure, loss of assistance, overheating, electrical symptoms, visible damage or change in vehicle behaviour. Include the checks, tests or diagnosis carried out and explain why the supplied item is believed to be the cause.

Diagnosis and Related Checks

Professional diagnosis

Include details of any garage or technician inspection, test results, fault codes, measurements or written diagnosis available.

Surrounding components

Explain whether related parts were checked for wear, seizure, restriction, contamination, incorrect adjustment, poor lubrication or another fault.

Installation rechecked

Confirm whether mounting bolts, connectors, pipework, fluid levels, torque settings, alignment and any required calibration were rechecked after the problem appeared.

Please Provide Clear Photographs

The suspected failure

Include close-up photographs showing any breakage, wear, leakage, distortion, scoring, overheating or other visible evidence.

The fitted position

Include wider photographs showing how the item was installed, including its mountings, connections and surrounding components.

Labels and identification

Photograph product labels, batch numbers, cast or stamped markings and any other identifying details on the item.

Keep the item in its current condition

Do not dismantle, repair, clean, modify or dispose of the suspected item unless this is necessary to make the vehicle safe. Keep all removed components, fragments, fluids where practical, packaging and related parts until the claim has been reviewed.

Keep All Relevant Parts and Records

  • Keep the item, its original packaging, labels, instructions and all supplied fittings.
  • Keep any related components that may have contributed to or been affected by the failure.
  • Retain fitting invoices, diagnostic reports, fault-code printouts, photographs and any relevant garage notes.
  • Do not return or dispose of anything until we have reviewed the claim and provided instructions.
